When a web pages changes, I sometimes see the changes highlighted, while on other web pages I do not.

The 'Highlight changes' feature of Active Web Reader is able to detect changes between the old copy of a web page and the new copy of the same web page. These changes are highlighted.
If you are accessing a web page off your hard disk then Active Web Reader does not maintain the old copy and hence is not able to highlight the changes.
