I've tried making a movie using the different output formats, but it always comes out as just the audio. The video is missing completely every time. Also, when using the WMV format, the wizard crashes immediately after the progress bar reaches (or seems to reach) 100%.

Any ideas what I could be doing wrong?

Upgrading Windows DirectX will solve this problem. All Windows upgrades are free from Microsoft.

Upgrade your Windows DirectX to DirectX 9.0c End-User Runtime.

If you already have the latest DirectX, I highly recommend you reinstall.

After upgrading, reboot your PC and make your movie again.
